ISS Individual Supports Services for Youth with Intellectual and/or Developmental Disabilities
Caring For Families Social Services has partnered with the New Jersey Children's System of Care (CSOC) as a Contracted System Administrator (CSA) to provide Individual Support Services for your child. We are committed to helping children with behavioral, mental health, or emotional challenges gain access to the services they need.
The New Jersey Children's System of Care (CSOC) is a division of the Department of Children and Families that provides an array of integrated and trauma-informed supports and services to youth with behavioral health challenges, intellectual and developmental disabilities, and/or substance use needs.

Individual Support Services (ISS) are provided in the youth’s home and/or in community-based settings. These services will support the acquisition and maintenance of goal attainment in home and community settings. The service will focus primarily on the use of Positive Behavioral Supports; instruction in activities of daily living tasks such as:

Eating

Toileting
-
Grooming
-
Dressing
-
Bathing
-
Transferring
-
Meal planning and preparation
-
Managing finances
-
Shopping for food, clothing, and other essential items

Performing essential household chores
-
Communicating by phone or other media

Traveling around and participating in the community
